How to realize multiple choice and single choice in the same table/matrix question

Hi, friends, 2.1 I don't know how to realize column A for multiple choice, and column B for single choice? 2.2 [Loop max of 2 hospitals; if >2 hospitals are chosen randomly allocate to 2, but must include the hospital where (B) was chosen] Look forward to your answers? ! ! Thanks

For the first point please try "side by side" question type. For the second point, I guess you will be able to achieve it using "Loop& merge" feature of Qualtrics.
