Is it possible to create a field for name that includes more than one field?

Hello all, Qualtrics newbie here. :) We are moving from Wufoo to Qualtrics, and I'm struggling with something that I hope has a simple answer. I searched, but couldn't find what I needed. In Wufoo, we had an "out of the box" feature for Name that would include space for the user to include Title, First, Last, and Suffix. Is it possible to create a custom field in Qualtrics to accomplish the same thing? Here's what I'm hoping to achieve: ! Thanks in advance for your help! Cindy
