I’d like to ask whether anybody has come across the following issue before. I have a survey with questions setup such as: ! ! However, the default report does not show the question fields correctly, particularly the last field. It appears the last field is shown which words from the previous field, e.g. ! ! Can anybody help?

I normally generate a new report when I'm having trouble with input to see if that resets whatever error is occurring. This is fine if I haven't already done a lot of work to the report but a pain if you already have done formatting.
