Is it possible to only display the 3 text boxes to the right if 'Used service?' has been ticked for that particular row? !

This isn't the answer you were looking for, but a possible solution without a need for custom code: Have you considered instead asking this as two questions, the second if which carries forward selections from "Used service"? I think that gets you the information you need, without burdening the respondent, but additionally might look better on more devices.
