Prepopulating a survey with permalink from library catalog | XM Community

I have a form question. It's probably easier to illustrate with examples. I'm a librarian, who works with our library software. I'd like users to be able to report problems with library records. I've created a Google Form that links from a library record and the form prepopulates with the permalink for the item record. We have half a million records and the form works with Javascript (see attached) to carry the permalink from the record into the google form.

Specifically, look at the "Report a Problem" link (see the triangle below the Request options). Click on the link. I've set up a Google form with the "prefilled link" feature so that the permalink fills in the permalink field in the Google form. I'd much rather prefer to do this with a Qualtrics survey, but I'm not seeing how I could do that. The closest I've found for Qualtrics is this https://www.qualtrics.com/support/survey-platform/survey-module/survey-flow/standard-elements/embedded-data/ And I tried doing this with my Qualtrics form, using this modified URL https://hollins.qualtrics.com/jfe/form/SV_2rCGW8giaF4Uo6x?permalink= But it didn't work when I made the changes to the library system. The permalink field in the form doesn't populate with the permalink.



The Google form works by using a custom Java script (see attached). I'm wondering what I need to change in the Qualtrics form and the Java script to get the Qualtrics survey to prepopulate with a permalink.



Any guidance is appreciated!
