Can't create a new action or add a task to an existing action

Hi All,
Is anyone experiencing an issue when trying to create a new action or add a new task to an existing action? I get a white prompt box that appears and then disappears straight away when clicking on add task. In the inspect window of Google Chrome I spotted this error message.

I've raised it with Qualtrics support but heard nothing back yet.
