QR Code for 360 survey

Let me start by asking a quick question: 
Why do you need to know who answered what? Each response will be given a unique Response Id so you can separate them. 

If you need to know their email address for validation (or any other reason) then you could add an initial block to ask the responders to state their email address before they can begin, but only show this initial question to those that come in to the survey via the QR code and not via email or phone.

Is it possible to generate QR code in EX survey in the Qualtrics ?

Hello again @sahana 

We don’t have the EX portion of Qualtrics so I wouldn’t be able to tell you exactly. 

But in the CX part of Qualtrics you open up your project, go to Distributions and on that page select “QR Code” and the unique code for that specific project is generated. 

Perhaps someone else who is better skilled in EX could jump in and let me know if it’s the same?
