I have a Radio Button with Values Yes and No.
The default value is set to 'Yes' . Let's say I select 'No' and close the window. When I re-open the survey page again , both yes and no radio buttons are selected. How do I clear the values and show either the default value or the last selected value.

I would recommend you clear your browser cookies and try again as if it is a radio button, it can have only one value selected at one time.
