Creating a Visual Analog Scale

I am trying to create a Visual Analog Scale that looks like the following:

I’ve reviewed previous posts, and it looks like I need to use a Constant Sum question with javascript.  The issue I’m trying to figure out is how to add the question above the lines.  For example, adding “How hungry do you feel” above the first line.
