I’ve created a survey and, for some reason, some of the questions will incorrectly record the response options chosen. For example, I will select “yes” but the data file will have the response recorded as “not sure” which is the third and final response option. We had previously determined that in-page logic was causing a similar issue (Recording the last response option for any question that had logic on it), but we can’t figure out what is causing the data to be misrecorded. Does anyone know what may be causing this?

Can you please let me know, where you are seeing this incorrect response recorded. Is it under Data & analysis?

What @Shasi mentioned could be a possible reason. You can check the recoding section to see if the variable naming option is enabled and validate that the naming values are correct, or even the recoding itself could be misconfigured.
Yoy can check it from each response and compare it with the results in the Data & Analysis section
