Question with Java Script - no recording in exported data

Hi Everyone, 

help needed,  I never before used Java script. 

I saved code to the workflow, but recording for Java Script is showing just for 1 out of 4 surveys (it’s a cross-cultural study, same measures, 4 different countries), I followed all same steps for all 4 surveys, any idea what is wrong and why I have recording just for 1 out of 4 surveys?
