Reporting on LIkert Statements Individually

Hi All,
Hoping one of your wizards out there can help me! I am working on a peer evaluation survey and in the past, we have had students fill out a separate form for each of their teammates. What I wanted to do was create 1 survey they could complete using likert scales for each of the questions. The Statements would be the name of each of their teammates. While this works well in theory, in the end I need to be able to create an individual report for each of the statements.
So, here is an example of one of the questions:
There are multiple of these for different dimensions. In the end, I need to be able to combine the data for each student to it's own report. Is this possible or do I need to go back to individual surveys for each team member?
