Is there a way to have the Gender part be a multiple choice type question within the Form?

For a survey question, I assigned the question type as Form under the Text option. I am trying to have respondents fill out Name, Age, and Gender for each of their kids. Is there a way to have the Gender part be a multiple choice type question within the Form so the data is easier to analyze?

There are a lot of ways to do this. I'd recommend looking at the "Side-by-side" question type for an easy user experience.

This was a great suggestion! I have never used this question type before. Do I need to have a description for both the column and answer? It's perfect for the Gender part but I just need one of them to say Name.
