Request to attach document to survey

Please I need help attaching a document to my survey where my respondents can make comments to the attached document as well 

Please is this possible? And if it is, how do I go about this?

Can you use the 'Descriptive Text' question type under 'Static Content' and then choose the type to be 'File' rather than 'Text'? That allows you to upload a file.

    Hello 

    Please I need to embed a document to my survey. I have attempted to upload these in the file's library. 
    Will these be shown in the survey when respondents fill out the survey because I will like the respondents to make comments on the attached document
