Responsive Dialog Creative wont display survey directly in the dialogue

So I've created a new Survey, then when setting up the creative and the intercept I chose a Responsive Dialog Creative, then set the Intercept to use this specific Creative and the new Survey.
I want the survey to appear in the creative but once published it still presents the creative with the two buttons to take the survey or decline. I've followed all the instructions on the support site but still no luck.
Is there anything else required other than checking the checkbox to "Display survey directly in the dialogue" on the Creative Size and Style section?
Just not clear what I've one incorrectly here.
