📂 QSF Template: COVID - Check in/out form for building entry

This is our check in/out form we are using at all sites for staff and visitors at our buildings. Every time someone comes to a building they have to check in via this form and check out when they leave. Staff/visitors can scan a QR code on site or use a laptop that is provided. The form is set to loop once completed. We are using this form to assist with contact tracing at our schools and other district facilities. Staff/visitors are asked to attest that they have no COVID symptoms and/or not in contact with someone with COVID. If they yes to any of these conditions they are asked to not enter the building and to go home. This survey is built into a dashboard that makes it easy for us to filter by day and location to check.
