We give this survey and a similar one to parents to ask about student and parent experience with teachers and specific classrooms/periods.
The data is used in teacher evaluations as well as school improvement projects.
What I like about this survey: There is a drill down for each school that provides current data on teacher, course and period, where applicable, so that students can talk about their specific class with a teacher. I really like that level of discretion bcs a teacher's 1st period ELA 10 class may be very different from their 4th period AP Lit class, etc. Other students in the room may have a lot to do with the experience, let alone time of day, etc. lumping all that under the level of teacher at best gives you mean scores, but rarely any actionable data.
What I think is cool and makes me feel proud about this thing: The 3rd block, Teacher Selection," is where the magic happens. Student tell us which school they attend in Block 2. An though Block 3 has a drill down for each school, students only see the school they attend. This series of drill downs allow me to get past the limits of one HUGE drill down for all schools, etc. Them using some LONG chains of piped data in the survey flow, I name their responses as TeacherName, CourseName and Period. That piped data is the responses for the appropriate place in all the drill downs, bus since they answer only one drill down, it shows only their response. Boo Yah! Magic.
What I would like to do moving forward: have out SIS do a nightly upload to Qualtrics with the class level data. I spend way too much time creating the drill downs found in the 3rd block, "teacher selection" and inevitably, I get a call from a school saying someone just took over for another teacher, or I forgot someone, etc. Thus I have Block 4. This is a clone of Block 3, but the questions are made invalid with some crazy GeoIP requirement, etc. But when I have to change a drill down, do so in block 4, make the question display when needed, make it's twin in block 3 no longer display in the survey using some additional crazy requirement, and hit publish. I do this because replacing a drill down for a question that has already been responded to can make things go very, very bad. Block 4 responses are also in those long chains for the embedded data terms in the survey flow. that work ahead of time has resulted in me being able to make quick changes when I get a call from a school and other than the initial error making me look like a dummy, I am otherwise coming off pretty pro... well, at least I'd like to think so. Maybe I should put out a survey... ;)
