Custom code works on question preview but not on full survey

I have inserted the following code into two survey questions to allow the respondent to add more "Other" rows to a matrix table, using a "Add Other" button. It works beautifully when I preview the individual question in survey builder, but does nothing when I preview the survey as a whole or use the anonymous link for distribution. Looking at the code, any ideas on why it doesn't work when I run through the whole survey?
