New Question Style Challenge

I am interested in piloting a new question type called ‘confidence weighted multiple choice’ and would love to use Qualtrics for this pilot. 

Below is an image of how it looks to the end user and how it is scored behind the scenes.  Does anyone have concept of how something like this could be done - if at all?
