We can add a custom validation as if (Not Sure is selected) OR (Text entry is greater than or equal to <minValue> AND Text entry is less than or equal to <maxValue>)

@harendra.kumar - Use a multi-select multiple choice question. Make ‘Not sure’ exclusive and add ‘Allow text’ to the first choice. Use JavaScript to hide the label of the first choice.
