Topic: Adding a Consent Form to a Survey

 

Consent forms in surveys typically consist of a text section explaining what the participant is consenting to and a multiple-choice question that allows the participant to decide if they wish to continue or exit the survey. 

 

Obtaining consent before a participant begins a study is critical, not only to be IRB compliant, but also to promote transparency, protect the participant, and ensure reliable data is collected.

 

If the participant decides to not take part, skip logic will take them to the end of the survey without viewing any other questions. This same survey style isn’t unique to consent forms, it can be used to make similar forms like Terms and Conditions, waivers, and contract agreements.

How to create the consent question:

  1. Go to your Survey Project.
  2. Add a new Multiple Choice Question with 2 choices.
  3. Change the answer choices to “Yes” and “No”.
  4. Add Skip Logic to the question.
  5. If “No” is selected skip to the End of the Survey.

Question of the Day:  Do you typically create a separate survey for your consent form and redirect respondents to another survey or do you add a consent block in the beginning of your survey? Why?

We haven't added consent form in most of our survey at the beginning  because then its like inviting someone to your house and asking them do you want to come 🤣😂😁

 

But we have added the question in the middle of the survey if participant wants to continue or not after our mandatory questions

Hello @krbhavya, I must admit that was a really good way to describe why sometimes consent forms can seem redundant. 😂 
Most of the time we see consent forms be used for student or some type of research studies where the participants have to be prompted so they know what they’re agreeing to before starting the study. 😆

Adding the question in the middle is a good strategy, thanks for sharing!

While there can be various approaches to take consent, we all must admit that this is an important step and it is necessary to follow this as a practice.

Hi JoseS, yes, as per the GDPR rule, if you are collecting PII information then respondent consent is must. Specially in European Survey.

We don’t take respondent’s consent in every survey. we only use this question whenever we record PII information of respondents using form/OE question in the survey. For Anonymous response, we never take consent.
