+1I need to create a custom page to add to my qualtrix survey that is basically 120 radio buttons with labels organized across rows and columns. I have html code but I need to add Javascript to activate the form in the qualtrix. how do I do that? See screen shot below.

Just type 120 in the number of choices and set alignment to column. 5-6 column is good enough
