Replace buttons with image and make clickable

Hi,

I’m trying to create a question that is similar to the example below. I’m really just looking for the statement, and a thumbs up and thumbs down option. They will have the option to select up to 5 thumbs up and 5 thumbs down. I’m not really sure how to replace the radio buttons with an image. I was thinking that the Bipolar Matrix Table question was the closest thing to doing this, but I’ve seen posts on the community about using the single answer multiple choice question. Any guidance would be helpful. Thanks.

@resh_kr I started going that route, and that may be the route I have to go if I can’t get everything I want to set up. Here was my attempt at it. Ran into some issues with trying to get the statement to line up in line with the thumbs up/down and the radio button.
 

 

What I’m hoping to do is replace the radio buttons so that the thumbs up/down take the place of those to select. We are close, but not quite there yet.
