It is not that surprising that it doesn’t work with Simple layout. Actually, I never recommend using the Qualtrics library date questions regardless of layout - the JS is dated and clunky.
Two alternatives:
- flatpickr
- html5

Thank you! I will try these approaches. My development team and I found the JS in the Library to be broken anyways. It is frustrating that this is the recommended approach in videos/documentation from Qualtrics when it does not work.

I asked around about this topic for you, and was able to find out the following:
“Some HTML and JS doesn't work with simple layout. While from a default state it works pretty well, when trying to alter the HTML with simple layout it can break. This is something our product team would need to investigate.”
