If you're creating it for the first time, it may take some time to load. I just tried it now and it took a bit of time to load for me as well, but it eventually loaded and is working fine. I've attached a screenshot for your reference:
In your case, the delay could be due to your internet network or a system lag. You could try deleting the one you created and then creating another one. Also, please check your internet connection. If the issue persists, consider raising a support ticket with Qualtrics.
