Hi -

At the end of my survey (which has two ending messages), I’d like to ask if the person taking the survey wants to submit their email for follow up information.  Is that possible?  How?

@elterrell I would suggest make an OE question and make it a non-mandatory at the end of the survey before the end message. And ask them to enter their email address. I believe this may help to solve the purpose.


EIther an open text entry or a form field question should be sufficient for this


Create a Single response question at the end and ask whether they are interested for a follow-up with Yes/No response.  If Yes, then in next page ask for Email address (mandatory), otherwise Skip to End.


Create a Yes/No question and set “No” as a default choice. If the customer has consent to follow up then they can update it as Yes and submit the survey. If not by default the survey response will be recorded as No.


Is there a way to put the form field question on the end of survey message?


@elterrell I don’t believe there’s a way to ask questions on the end of survey message, but you could place the question on a separate page right before they submit the survey.


Simply keep Y/N question toward the end of survey (before EOS elelement), keep on text question where you will take email, on this question apply display logic to say if Yes is selected on previous question display email question on the same page and you will be able to do the followup as needed.
