I need to be able to embedded Qualtrics at and of hour long training video. I also need to have the survey be able to retake until they pass. I am not sure how to embedded the Qualtrics at the end but I know how to do the scoring and redo part unless that embedded will throw that off.

as the survey is at the end of a YouTube video, I assume the survey is anonymous. In this case I would suggest you to embed a QR code:
