Approvals and Comments in Survey-Not sure if Qualtrics can help me with this?

What I am trying to do is create a form at the Dean’s level, that will need approvals and comments prior to being received by the Dean, and depending on what department is selected will determine where the form is routed for the first level of approval, and each level of approval needs to have the ability to provide comments for the Dean. Can I do this in Qualtrics?
