How to pick the best question format for a 'How important is' question?
So it is 1 question with a list of several (prepopulated) aspects that I wish respondents to score on an importance scale of 0-5 for them. I also want to provide the facility for them to add in 1 or 2 more aspects that are not already covered if they wish to add in new ones (with scores too). I want to capture their details (Name, Role, Company, email) on an optional basis.

You'd use a Matrix Table question type and can use the 'Automatic Scale Points' to choose important and unimportant:
