Changing the position of the response label

I think this issue is related to the theme, can you try applying some other theme. Let me know if it works.
+20If there is no option of changing the theme then can you please share the survey preview URL? we can look into it
You can create a new survey with a similar question and share the preview URL of it...(this to make sure you are not sharing the original project link).
