Text entry in multi choice questions

Any thoughts on how to solve for the scenario when you have a text entry option in a column multi choice question? It makes the buttons sit a little wonky (below). ! In the past, what I've done is anchored my "other" option to sit where "option 3" is (below), but curious to know if anyone else has another solution! !

How to give a "Don't Know" option?

How to add un-editable piped text?

I have a survey where respondents enter costs for this year and we want to display the previous year's values for comparison in the same question. Currently, I've piped that value into the question. However, this allows the respondents to edit that number, which we do not want. How can I display this information in a format that makes sense without making this data editable?

How to restrict who responds to my survey?

I am distributing my survey with the anonymous link, but because of methodology restrictions, I absolutely cannot have multiple participants from the same IP address. Is there any way to prevent someone with the same IP address from taking my survey twice with just the anonymous link?

I am putting together an "FAQ" landing page on my Vocalize dashboard to answer my dashboard viewer's questions. I want to include links to a few other pages that give a better explanation of our CX program, but I can't figure out a way to put HTML into a Descriptive Text widget. Any suggestions?
